Your physician or health care provider will fax us a signed order detailing the type of study needed and any clinical information that will help us to perform a tailored examination that is most appropriate for your condition.
In order to expedite your check-in, we ask that you bring your insurance cards and picture ID. If you are a new patient, or if it has been more than one year since your last visit, you will be required to present these cards again.
Our patient registration forms can be easily filled out upon arrival, or to save you time you can download the form below. Please fill them out and bring them to your first visit.
A HIPAA authorization form is a document in that allows an appointed person or party to share specific health information with another person or group. Your appointed person can be a doctor, a hospital, or a health care provider, as well as certain other entities such as an attorney.

We are in the process of adding additional companies. Please call if you do not see your insurance company listed to see if we are currently in network. We also offer an affordable self pay
option for high deductible plans or individuals without insurance.
If you have insurance, we will bill your insurance company and any secondary insurance companies for you. We recommend that you check with your insurance company prior to your exam to find out what percentage of the exam your insurance will cover.
If you do not have insurance coverage we request payment in full at the time of service. Programs are available for patients experiencing financial hardship.

Insurance Plans with Annual Deductible
We ask our patients with insurance plans with annual deductibles to pay a certain amount on the day of their office visit. As of January 1, 2025, this amount is $150 for
all patients unless your plan has a copay/co-insurance option. This amount may vary based on the service we provide. You can always reach out to one of our offices to
find out the actual amount you should expect to pay. These fees that we charge go toward paying down your out of pocket costs associated with the visit while also contributing
to paying down your deductible as a whole. Once you can show that you have met your deductible for the year, we only charge your copay/coinsurance amount.
We have implemented these point of service fees for several reasons:
1. Helps patients budget medical spending and spread out payments while they are still coming entirely out of pocket.
2. Every patient has a legally bound contract with their insurance company. By enforcing our policy and collecting the patient's financial obligation, we are doing our part to
enforce those terms.
We understand that rising medical costs and other struggles presented in the last several years from the coronavirus can make health care seem unaffordable for some. We are

Other POS (point-of-sale) Collections
We ask each patient to pay, from those who pay solely out-of-pocket to those who are insured and need to pay either a deductible, copay, or coinsurance amount. POS collections
also include prior balances or payment plan payments and can be paid via cash, money orders, and credit card and checks.
